Getting Started

   

• As a registered healthcare provider, you can link directly to the GenCAN interactive database to access patient status at any time, as well as enter and review patient data, anywhere that you have internet access.

• Internet remote access to the system enables physicians and pharmacists to:

• Review patient files. Access provides a real-time review of all patient WBC counts, neutrophils,
and blood draw dates

• Report both weekly and every-two-weeks.

• GenCAN Clinical Coordinators and staff are based regionally, well trained in the management of patients
on clozapine and can be reached 24 hours a day, every day across Canada. See our Contacts page for
full information.

Forms are provided for healthcare providers so you can:

• Register as a physician or pharmacist

Enroll patients – both those new to clozapine or those switched from another brand of clozapine.

• Gain information regarding Gen-Clozapine and the GenCAN program.

These forms indicate acceptance of overall responsibility for patients receiving clozapine.
Registration can not take place if all fields marked ‘must be completed’ are not provided.

Simply submit the Patient Registration Form with all the required fields completed.
All registered professionals can access GenCAN via fax, on-line or with a convenient toll-free number.

To complete this form you will need:
• Physician License Number
• Name of Physician
• Pharmacist License Number
• Pharmacy Name
• Primary Contact Information
• Patient Information
• Provincial Health Insurance Number
• Treatment location
• Laboratory

Hematolgical monitoring regimen is designed to provide minimum requirements for tracking results of patients receiving Gen-Clozapine.
(Bloodwork monitoring is conducted as specified in the product labeling, and after reviewing the patient’s clinical condition that blood counts are acceptable before confirming drug therapy.)

 

IMPORTANT – Both physician and pharmacist signatures must appear
on each patient registration form.

If all the necessary information is complete, the patient is registered. After registration activation, GenCAN will provide both the physician and pharmacist a Registration Confirmation. This Confirmation includes the Patient Identification Number which is a unique identifier specific to the GenCAN program.
