• As a registered healthcare provider, you can link directly to the GenCAN interactive database to access patient status at any time, as well as enter and review patient data, anywhere that you have internet access. • Internet remote access to the system enables physicians and pharmacists to:
• GenCAN
Clinical Coordinators and staff are based regionally, well trained
in the management of patients Forms are provided for healthcare providers so you can: • Register as a physician or pharmacist • Enroll patients – both those new to clozapine or those switched from another brand of clozapine. • Gain information regarding Gen-Clozapine and the GenCAN program. These forms
indicate acceptance of overall responsibility for patients receiving
clozapine. Simply
submit the Patient Registration Form with all the required fields
completed. |

Hematolgical monitoring regimen is designed to provide minimum requirements
for tracking results of patients receiving Gen-Clozapine. |

IMPORTANT – Both
physician and pharmacist signatures must appear If all the necessary information is complete, the patient is registered. After registration activation, GenCAN will provide both the physician and pharmacist a Registration Confirmation. This Confirmation includes the Patient Identification Number which is a unique identifier specific to the GenCAN program. |
